The service is committed to making homelessness rare and non-occurring.  
Te Kaupapa: Purpose   As a Housing First Key Worker you will work closely with your Team Leader to support the wellbeing of whanau and help them acquire the ability to maintain their tenancy and live well in their home. You will walk alongside whanau on their recovery journey as they build a sense of wellbeing and positive community connections based on their own choices and self-determined goals.

You will provide intensive, often long-term support based on these goals. You will work with a Property Manager to assist people into housing of their choice and provide ongoing support to participants as they establish themselves in a suitable home. You will continue to walk alongside people on their recovery journey as they build a sense of wellbeing and strong positive community connections. You will also have extraordinary inter-personal skills, patience and empathy to build trusting relationships with the Housing First participants and their whanauDuties | Mahi As a member of the team, you are responsible for acknowledging and enhancing the mana of the people we work with. You will provide intensive, often long-term support based on these goals. This includes providing a range of holistic support to promote wellbeing: 
SUPPORT WHANAU TO MAINTAIN THEIR TENANCY AND LIVE WELL IN THEIR HOME Work alongside property management to assist whanau into permanent housing and provide ongoing support to participants as they establish themselves in permanent housing. Identify needs and areas of concern of the whanau. Work with your Senior Key Worker to connect whanau to relevant agencies.  Ensure home/outreach visits are proportionate to their level of need. END TO END CASE MANAGEMENT Create and work towards whanau led goal plans.  Support the process of onboarding new referrals to the programme.  Continue to walk alongside whanau on their journey of recovery to achieve their goals, and to build strong positive community connections.  Documentation; Case Reviews; Contribution to whanau needs and risk assessments. COLLABORATION Work closely with your Team Lead to ensure core tasks are met. Connect with peers to share learnings and engage in best practise.  Attend multi agency meetings to connect whanau with appropriate support.  Build strong relationships with Airedale Property managers to reach shared housing outcomes with whanau.  Attendance for regular team hui. Skills & Experience | Pukenga & wheako Relevant qualification or experience in the social sector.  Experience in working alongside people with complex needs, who struggle with their mental health, substance abuse and/or trauma. Be motivated to work in this area to support people experiencing homelessness. Demonstrated ability to maintain professional integrity and self-care. Openness to learning and reflective practice. Awareness of harm reduction and trauma informed care. Excellent interpersonal and communication skills, with the ability to work autonomously with a strong work ethic. Ability to work respectfully with diverse cultures, communities, and people.  Openness to learn and/or cultural competence with Tikanga me te reo Maori. Computer literacy, basic record keeping, and ability to use Microsoft office applications.Knowledge of the client management system Recordbase is an advantage.   Team player A full NZ driver's license Job Vacancy close on 08th November 2024
