About the role The health coach works to support whanau on their health and well-being journey.Acts as a liaison between the patients and their dedicated GP and nursing team.Connecting patients with resources and other healthcare professionals to improve health outcomes.Working with patients with long-term illnesses encouraging positive lifestyle changes.Full-time role Monday - Friday.Benefits Free GP consultations.Ongoing Professional Development opportunities.Lovely friendly work environment.Skills and Experience Proven experience in a complex and changing Health or Social Services environment.A strong understanding of health services, and how to access them is ESSENTIAL.Experience working alongside whanau with a multi-cultural background.The ability to speak Te Reo Maori would be very beneficial.Excellent interpersonal skills and the ability to listen and empathise without judgement.You must have a Full NZ Drivers Licence.Ready to make a difference in the community?
