Uruuruwhenua Health is excited to invite applications for the role of Community Care Coordinator as part of the local Comprehensive Primary Care Team (CPCT).About the roleThis is a permanent, full-time position but we can offer flexibility for the right candidate. The position is based at our Uruuruwhenua Hauora, Frankton office, working across the local GP practices of Tahuna/Queenstown. This role will lead the CPCT team which includes a Kaiawhina/Community Support Worker.As Care Coordinator, you will play a pivotal role, focusing on delivering top-notch coordination to enhance the health outcomes of Whanau with complex health needs. You will be working with a team that values Whanau-centred care, making a real difference in the lives of those requiring seamless care across multiple providers. You will proactively monitor health, supporting Mana-Motuhake/self-management and preventing re-hospitalisations.
