Utilise the InterRAI tools to undertake assessments – current tools include Home Care, Contact and Long Term Care Facility Assessment. Maintain efficient and effective communication and identify and flag issues and risks in a timely manner. Assess clients in their homes and in a variety of health settings. Utilise the national assessment tool interRAI to assess and determine best outcomes for our client group. The assessment is a structured, comprehensive electronic tool that you will work through with the client in a conversational manner. Write comprehensive reports in digital systems. Create a plan of care that is client centred and goal focused including developing support packages in a holistic and culturally appropriate manner for older people who require long term support in the community. Play a key role in connecting people and their whanau with formal and informal support services in order to assist them to achieve their goals.About the LocationOur fabulous Waitaki district is known for its affordable living, spectacular landscapes, braided rivers, geological marvels, beautiful lakes, long beaches, steampunk sculptures, and of course the penguins! You will have experience working with older people in either a community, aged care or hospital environment. You will need an excellent knowledge of community resources and services that are available to the older person. It is essential that you have a current NZ driver licence. Competent with computers and able to perform the duties listed above to a high level. Competency with te ao Maori, tikanga, and te reo Maori or a commitment to starting your journey and taking ownership of your learning and growth.
