Working within a multidisciplinary team to support and assist people using Pathways' services to reach and maintain optimum wellness by providing effective support as well as education and clinical expertise. Build the use of health metrics, personal goals, and physical health checklists with a trauma informed lens.
You will build an exemplar activity wellbeing programme to help tangata whai ora realise physical health equity and outcomes.Bridge the gap in accessing natural community resources by building capability and self-determination through targeted group and individual supports.Focus on services that provide connection and activity to support wellbeing and reduce isolation.Be confident in assessing and providing advice on managing risk alongside our tangata whai ora to maximise positive outcomes.Work Hours:
You will report to the Team Coach who supports and leads the team to provide excellent care to our community. The role is a Monday to Friday 8.30am to 5pm. You will work across our services in the Wellington area.
About You | Mou
We are looking for someone who has:
Registered with the NZ Social Workers Registration BoardHolds and maintains a current Social Work practicing certificate and meets the requirements of any competency programme.Demonstrates a sound knowledge of community resources that may enhance whai ora/taiohi/whanau care.Actively communicates outcomes or progress.Demonstrated capability in working with Maori whanau.Closing date | Ra Katinga: 6 January 2025.
