Job DescriptionVaka Tautua Limited is seeking a dedicated and compassionate Social Worker/Clinical Support professional to join our Mental Health Services team in Wellington. As a Permanent - Full Time position, the successful candidate will play a crucial role in providing essential support to individuals facing mental health challenges. Key Responsibilities:
Conduct comprehensive assessments of clients' mental health and social care needs.Develop, implement, and review individualised care plans in collaboration with clients, their families, and relevant healthcare professionals.Provide therapeutic interventions and support to promote recovery and improve the overall well-being of service users.Offer practical advice, information, advocacy, and emotional support to clients and their families.Participate in multidisciplinary team meetings to contribute insights into clients' progress and well-being.Ensure accurate and timely documentation of client interactions, progress notes, and reports.Engage in continuous professional development activities to stay abreast of best practices in mental health support services.Main Tasks:
Assessing the social care needs of individuals experiencing mental health issues.Evaluating risk factors and developing risk management plans where necessary.Delivering evidence-based therapeutic interventions tailored to the unique needs of service users.Liaising with external agencies, such as housing associations or community support groups, to facilitate holistic care for clients.Promoting self-advocacy and empowerment among clients by assisting them in accessing appropriate resources and services.The ideal candidate for this role will possess excellent communication skills, strong empathy, cultural sensitivity, resilience under pressure, and an unwavering commitment to improving the lives of individuals dealing with mental health challenges. Desired Skills and ExperienceThe successful candidate must: have a comprehensive understanding of Pacific cultures;clear a Ministry of Justice check;have a clean, full NZ Drivers Licence.The successful candidate will have: a tertiary qualification in social work, nursing, occupational therapy or other relevant clinical qualification. professional certification and registration in accordance with registration board requirements (or working towards)one to three years' experience in the community social work setting;outstanding communication and networking skills;strong communication skills in English and one or more Pacific languages;conversant in Microsoft Office.Pay will be inline with relevant experience, qualifications and any pay equity rates that apply.
