Rehabilitation Coordinator

Strive Rehabilitation Manawatu is a community-based rehabilitation service providing quality rehabilitation for people with acquired brain injury.We are looking for a Rehabilitation Coordinator to be part of our team. This is a full-time position, although there is some negotiability with hours.Key ResponsibilitiesDeveloping individualised programme plans alongside clients, and assisting clients to reach their goals.Managing a caseload of clients and coordinating their programme at Strive.Liaising with a client's family members and primary care givers.Providing assistance with the daily programme, guidance and support using strengths-based practice.Working closely with funders such as ACC, and reporting on client progress and attainment.Contributing to daily notes and planning.Skills and ExperienceA qualified health professional (such as OT, Physio, SLT, Nurse or Health Worker with at least a level 4 qualification, Certificate in Health and Wellbeing).Neuro and/or disability experience preferred.Effective communicator, and a commitment to building rapport with clients.Able to tailor their approach to meet a client's needs and aspirations.Able to work effectively in one-to-one and/or group learning situations.Able to work collaboratively in a team.Why the Strive Rehabilitation Manawatu?APC fees covered (pro rata) if required/relevant.Opportunities for professional development.Be part of an innovative team.Be part of an organisation that is relational and community-based.A modern and cutting-edge approach to holistic rehabilitation.Please Note: Applications close on 30 October 2024.You can contact the Office at (06) 3546728 if you would like a Rehabilitation Coordinator Position Description.
