Nursing - Community, Maternal & Child Health (Healthcare & Medical)Registered Nurses required for Two Care Coordinator PositionsComprehensive Primary & Community Teams - National Initiative supporting General PracticeGeneral Practice in Paihia, Bay of IslandsGeneral Practice in Kawakawa Te Aratu o Ngati Hine (location Bay of Islands hospital)We are looking for dedicated health care professionals to fill our part-time Care Coordinator roles at two different locations, Paihia, and Kawakawa, Bay of Islands.This is an exciting opportunity under a new national initiative, where we are able to offer a part-time position for a fixed period and would suit someone who has experience and is familiar with navigating healthcare services, is comfortable working with patients and whanau, and is able to work as part of a multi-disciplinary team supporting general practice.This new initiative aims to support an increase in access to services and equitable health outcomes through integrating a range of roles including Pharmacists, Physiotherapists, Care Coordinators, and Extended Care Paramedics into existing general practices. This model, along with improving health outcomes, has a strong focus on expanding the skill mix available to primary healthcare teams and includes support for specialist pathways development for each of the roles. With a priority focus on Maori, Pacific and rural people, these teams are intended to help reach more of our at-risk communities and reduce barriers to accessing care.Mahitahi Hauora is a primary health entity underpinned by Te Tiriti o Waitangi. Our Kaupapa includes ensuring whanau and communities can achieve self-determined wellbeing, access to the services they determine they need, and live a long and healthy life. As the largest PHO in the Tai Tokerau region, we work collaboratively with community, primary healthcare providers, and key partners to support general practices, Maori health providers, and social sector agencies in achieving better health outcomes for our Tai Tokerau population.What is the Care Coordinator Role?The primary purpose of this role is to facilitate high-quality coordination of care for people and whanau with the greatest complexity of care needs, particularly those who require frequent transfer between providers and across a variety of services.Working closely with the General Practice interdisciplinary team (IDT), you will be actively involved in meetings and using the systems and processes that support IDT functioning and outcomes. A key purpose of this role is to support increasing the capacity for other members within the general practice interdisciplinary team, with key functions that include:Facilitation and timely completion of information to enable resource alignmentHigh-quality care coordination for individuals and whanau navigating multiple services.Assessing and reviewing individual and whanau health and wellbeing needs.Providing tailored support and education to improve individual and whanau self-management of wellbeing.Ideally, you will be person/whanau centred with an understanding or use of a whanau ora approach, and a focus on reducing health inequities. You will be driven to empower individuals to manage their own health needs and be comfortable in working across multiple health sectors and services.Successful applicants will need to meet the following criteria; however, we are interested in hearing from healthcare professionals with a range of experience levels and backgrounds.Can you demonstrate the following:Primary or Community healthcare experienceWhanau centred, socially competent, and experienced in the healthcare service and system navigationDemonstrate cultural competence and understanding of Te Tiriti o Waitangi in actionComprehensive understanding of the inequities in access and use of medicines in Aotearoa New Zealand and its causesAdvanced written and verbal communication skillsExcellent time management and organisational skillsExcellent critical appraisal skills, being able to identify the best evidence-informed solutions to clinical and practice questions and issues.You must be a NZ Citizen or Permanent resident or have a current open NZ work visa.
