Practice Nurse

We are currently seeking an enthusiastic Registered Nurse to join our team on a part-time basis. Experience in general practice is essential.
Key responsibilities include but are not limited to;
Performing health assessments Prepare Care Plans and Team Care Arrangements to help patients manage their disease/s Administering vaccines Post vaccination observations and monitoring Triaging patients and prioritization of emergencies Assisting GP with minor procedures Managing the recalls and reminders system Cold Chain Management Coordination, management, and maintenance of patient clinical data Wound dressings and removal of sutures Health education and coaching to support patients with the achievement of their goals Manage administrative tasks relating to carrying out CDM appointment Assist in the early identification of chronic disease by conducting health assessments To be considered for this position, you must have:
Current AHPRA Registration. Minimum 1-year experience in a General Practice setting Proven prior experience in Chronic Disease Management and health assessments Excellent communication skills and working in a team environment Skilled with use of computers and practice management software (Medical Director PracSoft desired) A passion for people and a personable approach Compassion and professionalism Proven multi-tasking, problem solving and time management skills What's on offer:
