5 weeks annual leave after two years.
Sick leave entitlement of 5 days will be available from commencement of employment and another 5 days at 6 months of employment. 10 days entitlement will be available at 12 months service thereafter.
Position Purpose:
To be the liaison between WellSouth, patients and community services. This role will interact by phone calling community providers and patients to ascertain patient information, create enrolment forms, make follow up calls, populate current systems with newly gained updated patient data, triage and refer people to services.
