Salary: NZNO Nurse Practitioner Salary Scale Grade 6 Step 1-4 dependent upon experience and qualifications - Pro rata Korero mo te turanga - About the role We have an exciting opportunity with an innovative Matanga Tapuhi Nurse Practitioner (NP) Health of Older People or a Nurse Practitioner (NP) candidacy for this area of practice. This cross sector role based at Hutt Hospital improves access to comprehensive gerontology assessment (CGA) and coordinated interventions to support older adults to live safely in their own homes. To improve equity, the Matanga Tapuhi NP will work with Maori and Pacific people aged 55 + years who present to the Community ORA team with diagnoses usually associated with older people that impact their function and social well-being. One of the key objectives of our wider service is to support older adults to live safely in their own homes as long as possible. The NP role will contribute to this service objective by providing a timely, flexible and collaborative service with links to primary care. The NP will assist with identifying acute or emergent issues and offering treatment guidance and care to prevent further deterioration or loss of function. We are looking for an autonomous practitioner but you will not be working in isolation as the role is embedded with the Community Older Adults, Rehab and Allied health team while working closely with our Geriatricians and Frailty Registrar. Mo to matou ropu - About our team Te Herenga Tangata is our Community Older Adults, Rehabilitation and Allied Health service. We are a large, friendly, interdisciplinary team offering you access to Physiotherapists, along with Social Workers, Occupational Therapists, Nurses, Speech Language therapists, and Dietitians colleagues. Across the broader Community Older Peoples service there are other NPs and Senior Nurses. You will also have support from the Nurse Director and linkage with the Nurse Consultant Advanced Practice. Given that we are now part of a single District with Capital Coast you will also be linking with NPs working within the CC Community ORA teams. We are continuing to build relationships with Primary Health and a broad range of government and community providers. This collaboration is already making it easier for our staff to provide timelier, effective and equity focused health services. You will be involved in the ongoing evolution of these collaborative partnerships as we develop our practice to respond to the diversity of the Hutt Valley. Mou - About you We wish to see applications from people who are passionate about the health of our Older Adults population and committed to models of practice that will support the wider health system. You must be open to innovation and collaboration. The person in this role will: Be registered as a nurse practitioner with the nursing council or expected to complete a recognised nurse practitioner programme in the next six months. Have some experience of community OR acute health of the older person Excellent communication and interpersonal skills and the ability to work effectively with all levels of staff in a multidisciplinary team. A strong patient care focus and able to deliver high quality of care for patients and whanau in their homes. A passion for ongoing learning, professional development and the ability to share, support and develop others. Or if applying to start a Nurse Practitioner Candidacy: Registered Nurse (RN) and a current practising certificate with no restrictions from the NCNZ Expert level on the PDRP or Senior Expert pathway with at least three-years relevant expertise and working at an advanced RN practice level A Clinical Masters or the postgraduate courses to meet the requirements to apply for the national Nurse Practitioner Training Programme (NPTP). The District have a well-developed Nurse Practitioner Candidacy Programme.
