Kaitautoko Hapori/Community Health Coach As a Maori health and wellness service working across Te Tauihu with an emphasis on pae tawhiti /excellence, our vision at Te Piki Oranga is to Revitalise whanau for their future - Me whakahaumanu te mana o te whanau a, ka haere whakamua. About this role Te Piki Oranga is looking for a Health Coach/Kaitautoko Hapori based at the Stoke Medical Centre 32 hours per week. The Health Coach will support whanau with the motivation and capability to better understand and self-manage their physical and emotional wellbeing needs. They may also bridge the gap for whanau to access healthcare support related services. Whanau will be referred to the Health Coach by the clinical team, particularly the Health Improvement Practitioner (HIP) with whom they will work closely. The purpose of this role is to: Provide a support service that is accessible, integrated, coordinated and responsive to whanau. Liaise, facilitate and advocate for improved health outcomes with appropriate organisations on behalf of whanau within the practice. Specific 'Health Coach' training for the successful applicant is to be undertaken upon starting in the role. This is a key role that contributes to improving health outcomes for Maori, providing evidence-based services alongside a cultural model of care that promotes, improves and empowers best health outcomes for whanau. Our ideal candidate will have: Knowledge of the health sector, health privacy and health & disability rights. Experience working within the Maori community. The ability to engage with at-risk whanau and whanau with complex health and social needs – from a wide variety of backgrounds in a mana enhancing style. Well-developed and effective communication skills. The ability to handle situations effectively and respectfully. Ability to work independently. Competent computer skills. An understanding of how Te Tiriti o Waitangi applies to health and social services is desirable. Passion to support whanau to access effective and culturally appropriate services. A full driver's license is essential. Auahi Kore (Smoke Free) and able to model a healthy lifestyle. This Kaitautoko Hapori/Community Health Coach will be based primarily in the community with some time spent within our Te Piki Oranga Waimeha office.
