We're seeking a welcoming and capable individual to support our whanau with understanding their health journey and helping them assess all the available care and services they need.About Us:Te Taiwhenua o Heretaunga (TToH) is a whanau focused, kaupapa Maori values based, health and social services organisation providing professional health services to our local Hawkes Bay community and wider Kahungunu rohe from Wairoa to Wairarapa.
Our health, social, education and community development teams have been working successfully with whanau for over 39 years.Key Responsibilities: Engage with hard-to-reach whanau, encouraging them to reconnect with Hauora.
Collaborate with clinicians to gather feedback on whanau needs.
Conduct basic health checks and assist with clinical decision-making.
Ensure priority populations receive funded care.
Provide navigation support for follow-up care and service access.
Build relationships with whanau and the community, offering support during consultations.
Complete home visits, various checks and deliveries.
Help whanau sign up for the Hauroa My Indici portal for better access to their records and test results.

Full-time position (40 hours/week, Monday to Friday, 8:30 AM – 5 PM).
Ideal Candidates Will Have: First Aid Certificate Full Driver's License NZQA Level 3 or 4 in Health and Wellbeing (or similar) Warm, engaging personality with excellent people skills Experience in medical or dental administration (preferably with Indici) Professionalism and high standards of client confidentiality Understanding of tikanga Maori (or willingness to learn) Explore our workplace culture on our website or Facebook.
