Permanent, full-time or Contract basedLocated in Levin in the beautiful ManawatuPatient-centred care that addresses health needs and promotes wellnessHorowhenua Community Practice (HCP) is owned by THINK Hauora (PHO).  HCP is the largest general practice currently operating within the Horowhenua District with over 6 000 patients and is also the site for the centralised Afterhours Clinic for weekends and public holidays.

About the Practice The demographic mix of the practice patients is varied. There is a large population of elderly retired people who reside both privately and within the five main rest homes in the area. There is also a large Maori population, comprised mainly of Ngati Raukawa and Muaupoko. 
Levin is mainly a service community to the surrounding farming areas and is centrally situated between Wellington and Palmerston North.
Primary Function of the Role: To provide competent patient-centred care that addresses health needs and promotes wellness. Such care is based on latest evidence and guidelines and meets legislator requirements
