Community Mental Health Nurse Continuing Care TeamAn exciting opportunity has arisen for an experienced Registered Nurse or Occupational Therapist to join our Continuing Care Team (CCT) in the Adult Community Mental Health & Addictions Service.About the Role* Part time (0.8 FTE available in total)* Up to 32 hours per week, working business hours (Mon-Fri 8am to 4.30pm)* Provide specialist intervention for people to optimise recovery and quality of life as well as case coordination.* Competent in all facets of assessment, holistic care and multi-disciplinary teamwork and have demonstrated ability to work with all ethnicities, particularly Maori/Iwi.* To provide community mental health care based on current best practice and collaborative working within the inter-disciplinary team.* Ensure all inter-disciplinary referrals within the team will be performed with consent from whaiora and their whanau (where applicable).* Work collaboratively with complementary internal and external partner organisations.About You* Registered Clinician with experience in a community mental health environment.* Sound assessment and psychosocial intervention skills.* Annual Practising Certificate & Registered with the appropriate regulatory body.Salary RangeAs per relevant SECA (Single Employment Collective Agreement), based on qualifications and prior experience.How to ApplyIf this sounds like you, apply online now via https://www.lakesdhb.govt.nz/.
