Community Long Term Conditions Nurse

Position Overview: We are seeking a dedicated Community Long Term Conditions Nurse to provide holistic care to whanau.
In this role, you will focus on managing chronic conditions such as, but not limited to, diabetes, cardiovascular, and respiratory diseases, while ensuring equitable, culturally appropriate services are accessible to all.
You'll play a critical role in delivering care plans, facilitating self-management, and advocating for Whanau health outcomes.
Key Responsibilities: Conducting comprehensive health assessments and whanau focused care plans.
Ensuring integrated and culturally appropriate services.Provide evidence-based clinical care in collaboration with healthcare providers, individuals and whanau.Lead LTC prevention and management programs, including rehabilitation and patient education.Support discharge planning and transitions of care.Deliver self-management programs to support whanau empowerment and health literacy.Qualifications & Skills: Registered Nurse with a current Annual Practicing Certificate.Minimum of two years of experience, particularly in LTC, community or Maori health.Full driver's license.
