Uruuruwhenua Health is excited to invite applications for the role of Community Care Coordinator as part of the local Comprehensive Primary Care Team (CPCT).
About the role
This is a permanent, full-time position but we can offer flexibility for the right candidate. The position is based at our Uruuruwhenua Hauora, Frankton office, working across the local GP practices of Tahuna/Queenstown. This role will lead the CPCT team which includes a Kaiawhina/Community Support Worker.
As Care Coordinator, you will play a pivotal role, focusing on delivering top-notch coordination to enhance the health outcomes of Whanau with complex health needs. You will be working with a team that values Whanau-centred care, making a real difference in the lives of those requiring seamless care across multiple providers. You will proactively monitor health, supporting Mana-Motuhake/self-management and preventing re-hospitalisations. You will also facilitate interdisciplinary functions at the General Practices, collaborating with health and social service providers.
What we are looking for
A Registered Nurse, Occupational Therapist, Social Worker, or other registered health professional with a current practising certificate.
Ability to thrive both independently and in a collaborative team environment.
A positive "can do" attitude with a commitment to achieving the best health outcomes.
Strong cultural competency and confidence with sufficient knowledge of the peoples, languages, and cultures that form our most at-risk communities.
All applicants must be legally entitled to live and work in NZ and have an understanding of the principles of Te Tiriti o Waitangi.
