We're looking for someone who shares our dedication - supporting the elderly, people with disability and people rehabilitating from injury and illness to live independently in their own homes and communities.What does an RHP do?We carry out social and clinical assessments, develop and oversee support plans and mentor and verify the competence of our support workers.
