About this role
The Clinical Nurse Specialist (CNS) Vascular Access and Outpatient Antimicrobial Therapy (OPAT) is a unique role that works across the care continuum.
You will be responsible for case management of outpatients on intravenous and/or oral antimicrobial therapies and vascular access management, education, training, and support (inpatient and community) within Te Whatu Ora CM Health.
Some of the essential work you will be doing:
Work within the multidisciplinary team (MDT) to provide a cohesive patient-centred nursing service in antimicrobial therapy and assess patients to ascertain they, whanau, and carers can safely manage at home.
Provide training to patients (as applicable) and practical and emotional support to patients/whanau undertaking care.
Coordinate the discharge of patients to the OPAT service, case manage their ongoing care, and provide a telephone on-call service Mon-Sun ******** for queries, troubleshooting, line issues, and monitoring IVA related reactions.
Lead and improve the quality of professional nursing practice in relation to vascular access management by training staff to care for and manage vascular access devices (VAD).
Assess the vascular access needs of patients and recommend the most suitable vascular access (act as patient advocate in respect of patients' vascular access and intravenous therapy needs).
Provide onsite support Mon-Fri for staff and inpatients with vascular access devices (VAD).
Contribute to and/or develop quality activities related to vascular access, which include wider initiatives within the team and CMH Health.
Monitor standards of practice.
Educate and support health professionals across the health sector.
