Clinical Nurse Specialist - Neurology / StrokePermanent (0.4FTE)An exciting opportunity to work across two specialty areas, neurology and stroke, has arisen for a senior nurse with a passion for the coordination of patient-centered care and continuous quality improvement.Mo koe / About youWe are looking for a dynamic, self-motivated individual to join our team. You will have proven clinical abilities demonstrating a clear evidence base.Your leadership attributes will be evidenced by your work history, post-graduate qualification(s), and commitment to ongoing professional development.You will demonstrate excellent communication skills, a willingness to be flexible, and the ability to work autonomously and as part of a team.Mo te turanga / About the roleKey components of this dual role are the delivery of timely, effective, individualized care to patients and confidence in facilitating nurse-led clinics.Patient cohorts include those with neurological conditions such as Motor Neurone Disease and Multiple Sclerosis, in addition to other complex neurological conditions.The stroke nurse specialist component of this role will see you attending code strokes; stroke patient identification, ongoing review, goal setting, and discharge planning; providing patient and whanau education and support; data collection and reporting.Both roles include working in the hospital setting and community, ensuring coordination and integration of services to ensure quality of care and an improved patient experience.Awhe utu / Salary range$116,040 to $127,674 pro-rated to hours worked.If this sounds like you, apply online now via https://www.lakesdhb.govt.nz.
