JOB DESCRIPTION Job purpose To organise high quality clinical care for the Hummingbird House residents through: Outstanding clinical leadership. Efficient use of clinical and human resources. Ongoing quality improvement. Effective liaison with internal and external stakeholders. The Clinical Manager (CM) provides appropriate delegation and direction to Registered Nurse/s, Enrolled Nurse/s and to the Healthcare Assistants to work effectively as a multi-disciplinary team. CM will practice in accordance with the partnership inherent in the Treaty of Waitangi. The approach to care will be client-centered and holistic – this will be reflected in the care planning process focusing on the choices and needs of individual residents. CM will provide professional nursing care to the residents, optimising their health, level of function and wellbeing and as well provide support for the family/whanau. Core Competencies Maintains a comprehensive and accurate assessment of residents using suitable assessment tools. Facilitates care planning, involving the resident and/or their representative, demonstrating through the care plan an understanding of residents' rights to make informed decisions. Ensures residents are provided with appropriate information to make informed decisions relating to treatment and care reflects client preferences in all areas of service delivery. Provides an innovative and flexible approach to complex care planning. Monitors residents' care interventions for their effectiveness and minimises any variance. Practices nursing professionally and demonstrates accountability for own scope of practice in accordance with the Nursing Council Code of Conduct. Facilitates comprehensive infection control programme and ensures it is consistent with Hummingbird policies, procedures and up to date direction from authorities. Effective leadership Brings to the role and to the team a mindset of excellence and improvement. Communicates with staff members to facilitate and achieve a team approach in all areas of care provision throughout the facility. Actively participates in staff training and orientation. Allocates human resources according to relevant policies, standards and guidelines ensuring that immediate care requirements are met. Manages all resources and products in a cost-effective way – particularly continence products, gloves, wound care products and other medical supplies. Informs the financial manager of any clinical care that could have a cost implication. Identifies areas of operation where efficiency can be improved. Creates and executes improvement plans. Communicates and interacts internally and externally in a professional manner. Practices active listening by demonstrating the intention to hear and understand another person's point of view. Creates an environment that encourages others to work together. Maintains a professional and positive relationship with Health New Zealand, NASC Team, Medical Staff and all medical practitioners. Controlled demeanor Maintains a positive, cheerful attitude. Communicates effectively with people of various cultural backgrounds and understand their unique perspectives. Is tactful and sensitive, particularly when discussing care requirements and 'advance directives' (in relation to end of life questions) with the frail elderly and/or their relatives. Treats residents with respect and dignity at all times. Health Practitioners Competence Assurance Act 2003 You are required to maintain your current competency-based practicing certificate. You must notify the employer of any changes to scope or conditions of practice (determined by regulatory authority). You must complete the requirements of any competency programme. You must notify the employer of any concerns relating to the risk of harm to the public of another health practitioner below the required standard of competence. You are required to know the provisions of the HPCAA as the governing legislation. Core Functions and responsibilities Screens potential residents to ensure appropriate placement and to determine existing and potential risks. Assist Facility Manager to ensure all admissions are welcomed and settled in satisfactorily. Ensures all necessary admission documentation is completed. Supervises scheduling and daily assignment of duties for nursing staff. Ensures that initial care plans are developed within prescribed timeframe and with input from the resident and their family. Ensures that InterRAI Assessments are completed and comprehensive care plans are written for each resident and that these are evaluated at least six monthly or when changes occur. Ensures that care staff adhere to the care plans, which may include such things as ambulating, continence management, and measures to avoid physical, mental, and social deterioration and other matters relevant to the individual. Advises on care and administration of medication and ensures that staff understand side effects of the medications administered. Ensures staff competence is tested and signed off. Provides and supervises care of residents; particularly those who may need specialised nursing care, for example, a regular injection, wound dressing etc. Delivers and ensures safe, efficient, and therapeutically effective nursing care by good prioritisation of personal and collective workloads. Respects the rights, autonomy, privacy, and dignity of residents. Ensures roster changes are made as needed to accommodate resident need and volume. Prepares and maintains an on-call schedule. Ensures residents are seen by an emergency doctor or go to hospital if required. Maintains smooth operation of all care services within the facility. Ensures that all residents have their nutritional needs met. Ensures all residents are seen by GP at least three monthly and their medication charts are reviewed. Ensures that relatives are informed of any changes resulting from GP visits. Maintains accurate clinical records and ensures privacy and confidentiality of information. Coordinates with multidisciplinary team members to ensure their input into care plans. Ensures the residents' care plans consider people's life experiences and choices; maximises independence and self-help abilities; involves family / whanau; includes their support networks and takes into account the physical, mental, social and spiritual needs. Ensures that ongoing assessments and reassessments of the residents' health status are completed. Actively seeks professional development opportunities and attends at least 8 hours of clinical management training annually. Ensures efficient and safe usage of supplies and equipment. Ensures that care staff are appropriately orientated and inducted. Deals effectively with enquiries relating to the residents' health status. Participates in quality improvement activities to monitor and improve standards of nursing. Participates in annual performance appraisals for care staff. Partakes in staff recruitment and interviewing. Ensures that there is a designated Infection control and restraint coordinator. Assists in developing and implementing programs to keep nursing staff informed of new trends in nursing field. Ensures doctor is notified of any new admissions and organise the new resident to be seen within 2-5 working days after admission. Communicates with relatives and documents changes or updates in care plans, incidents, GP outcomes, resident unwell. Ensures the resident has adequate explanation of the effects, consequences, and alternatives of proposed treatment options. Conducts nursing practice in accordance with professional, ethical, cultural, and relevant legislated requirements. Making professional judgments that will enhance nursing practice. Demonstrates ability to apply the principles of the Treaty of Waitangi. Recognises the impact of the culture of nursing on care protecting the resident's well-being within this culture. Practices in a manner that respects each person's identity and right to hold personal beliefs, values, and goals. Takes accountability for directing, monitoring, and evaluating nursing care that is provided by enrolled nurses, caregivers and others. When delegating work, takes into consideration the roles and competencies of staff. Promotes an environment that enables resident safety, independence, quality of life and health. Identifies, reports, and investigates adverse events. Follows up on outcomes of adverse events and helps to prevent their recurrence. Acts appropriately to prevent and/or manage emergencies and disaster situations. Monitors the environment for potential hazards, and reports these. Accesses, maintains, and uses emergency equipment and supplies. Manages challenging behaviours to achieve optimum outcomes for all parties. Fulfills the responsibilities as outlined in all service contracts with Health New Zealand, the Ministry of Health, ACC etc. Ensures that Section 31 notifications are made without undue delay. Prepares and maintains all documentation to the highest standard for internal audits as well as certification and surveillance audit purposes. Makes recommendations to the Facility Manager on issues such as reviews of internal and external developments/documents and provides guidance on compliance or recommendations for change.
