As a Case Manager, you will be the face of MSD, engaging directly with individuals and whanau to provide financial support, employment services, and housing assistance. You'll support people to achieve their goals by connecting them with the right services and creating tailored plans that work for them.
What you'll be doing | He aha tau e mahi ana Building strong relationships and working collaboratively with others to enhance outcomes for whanau.Developing and maintaining relationships with your peers and partner agencies to give and receive support when needed.Engaging with individuals, employers, and stakeholders to identify barriers and create plans for success.Providing empathetic and professional case management support, maintaining accurate records of client interactions.Managing client progress, adjusting support plans as needed to help them achieve their goals.Navigating various computer systems to manage client information and stay up to date with relevant policies and legislation.Minimum Requirements The ability to work independently and collaborate with team members to assess client needs and develop tailored support plans.Experience in a people-centred environment or previous job where you have built connections, resilience, and a passion for making a positive impact in the lives of others.You will be organised, with sound decision-making and problem-solving skills.You will bring a high level of personal integrity and initiative and be able to take ownership and responsibility to follow through and deliver for the people in our community.We are seeking motivated people who are passionate about helping people from diverse backgrounds, and who have an aspiration for others to flourish, whilst being immersed within the community.Your ability to adapt and adjust support plans based on client progress will be key in achieving meaningful outcomes.Be purposeful: You will show care and warmth, give support to others.
