Te Mahi/About the Position
Case Managers have an essential role in community care, liaising with clients and whanau to assess clients' health needs, and establish and review care plans. You will also work closely with others involved in the homecare team to ensure our services are delivered to a high standard, and with other health services involved in clients' care and support.
We have a part-time permanent position available, usual hours of work will be Monday to Friday within business hours.
Key Aspects:
Providing comprehensive assessments of new and existing clients
Developing care plans in collaboration with clients and their whanau
Undertaking assessments and regular reviews of clients' needs, making sound clinical decisions
Documenting all assessments and outcomes accurately
Promoting awareness and acceptance of the assessment and care planning process
Working closely with the referrers, service coordinators, and service management to ensure proactive communication of any questions or concerns

To be successful in this role, you will have:
Strong people skills, with an ability to build rapport with clients, whanau, and internal staff
Superb organisational and administration skills
A proven ability to prioritise and manage workload effectively and efficiently
Experience working in a community-based healthcare environment
Flexible and positive attitude towards change
Previous experience with comprehensive clinical/social needs assessments or care management would be favourable, as would a post-graduate qualification in a health-related field.
You must hold a current practicing certificate as a registered health professional, a full New Zealand drivers' licence, and the right to legally work in New Zealand.
