Undertakes all components of case management, including:comprehensive assessment, risk assessment and management, recovery, andtreatment planning
Provides client-centred care, utilising evidence-based treatmentstrategies including, where appropriate, talking therapies
Take part in the local crisis response service roster.
Monday to Friday
We are looking for a Case Manager with a Nursing or Allied Health background, who has the clinical skillsrequired to assist people with current or ongoing mental health problems and tomaintain their place in the community. Thisrole will be working within the community and within our integrated model ofcare.

Excellent assessment and communication skills
Flexibility to work autonomously as well as part of amulti-disciplinary team
Understanding and application of the principles of the treaty ofWaitangi, with a culturally safe and appropriate approach to practice
Commitment to ongoing professional development
Astrong understanding of Maori health inequity and Maori population need
Understandingof contemporary approaches to mental health and wellbeing
NZ Registered Health professional as a Nurse, Social Worker, Occupational Therapist, Psychologist or Physiotherapist.
