Medical Administration (Healthcare & Medical) Full time – 0.8-1.0 FTE (32-40 hours) – fixed term 12 month We are excited to introduce a new role of Care Coordinator at Taradale Medical Centre. You will need to be a Registered Nurse. A new role aimed at enhancing health equity by expanding service access through innovative roles within existing general practice teams and supporting our General Practitioners/Nurse Practitioners with clinical administrative tasks through inbox management.
This role is on a fixed term 12-month agreement.
About the role: As a Care Coordinator, you will play a crucial role in supporting our General Practitioners (GP) / Nurse Practitioners (NP) with their inbox management and facilitating coordination of care for individuals and whanau with complex healthcare needs. This role is for a Registered Nurse. The role aims to drive more equitable health outcomes for our patients by increasing access to services. Duties will include:
Support GP's/NP's with their inbox management and other clinical administrative tasks, which will support the delivery of a patient focused service Communication with patients and whanau on results, health outcomes or follow ups Collaborating with multidisciplinary teams to develop and implement personalised care plans for patients/ whanau Manage Recalls: Remind patients about upcoming health checks and screenings to ensure they get the care they need on time Advocating for patients and their families to ensure their goals and needs are met Providing support and guidance to navigate the health system Continuously assessing, reassessing and reviewing patient needs to optimise care Reduce recurrent patient/GP consultations as appropriate Collaborate with relevant health and social service providers and maintain relationships within the practice team, wider community, external agencies, NGO's and secondary care providers Ongoing training, development and support About Taradale Medical Centre: Taradale Medical Centre is a Cornerstone and Foundation accredited practice comprised of General Practitioners, Nurse Practitioner, Nursing and Support staff. As a member of the Hawkes Bay PHO, our goal is to provide safe, effective and quality health care to our patients. We are a busy practice offering a range of services including general consultations, diabetes and chronic conditions management, women's health, smoking cessation, nurse led skin check clinics.
