Access Community Health is a leading home-based healthcare and support provider. Our nationwide team of over 3,500 Support Workers and Registered Health Professionals reach over 40,000 clients | tangata in our community. We provide nursing, personal cares, rehabilitation, social support and household assistance to ensure our tangata are safe, active, and independent in their own homes. As a Care Coordinator for Access Community Health/Uru Atea, you will play a vital role in: Coordinating home-based care and support services that enable Access Clients to live as safely and as independently as possible within their homes and communities. Develop and maintain rapport within the Access team, our Clients and their Whanau. Deliver high quality, customer-focused service within a structured framework and in a highly flexible service response environment. Adhere to our policies and procedures to ensure our clients and support workers receive the best outcomes. Be responsible for coordination of support worker placements to deliver high quality care and support to our clients. Minimum Requirements: Eligibility to work in New Zealand. Commitment to providing excellent customer service. Excellent interpersonal skills that enable the building of effective relationships across diverse groups, including negotiation and conflict resolution skills. Proficient with general computer skills, including Microsoft Office suite. Excellent organisational skills, particularly time management and prioritising. An eye for detail and an ability to maintain a high level of accuracy. Ability to think creatively and strategically and utilise problem-solving skills. Work well under pressure and multi-task across functions and activities. A demonstrated commitment to Te Tiriti o Waitangi in the workplace. Actively demonstrates the ability and desire to show cultural sensitivity, awareness and understanding of diversity.
