We are looking for a dedicated health care professional to fill our part-time Care Coordinator role in Te Ara Tu o Ngati Hine, General Practice located at the Kawakawa Hospital.
This is an exciting opportunity under a new national initiative, where we are able to offer a part-time position for a fixed period. This role would suit someone who has experience navigating healthcare services, is comfortable working with patients and whanau, and can work as part of a multi-disciplinary team supporting general practice.
This new initiative aims to support an increase in access to services and equitable health outcomes through integrating a range of roles including Pharmacists, Physiotherapists, Care Coordinators, and Extended Care Paramedics into existing general practices. With a priority focus on Maori, Pacific, and rural people, these teams are intended to help reach more of our at-risk communities and reduce barriers to accessing care.
