As a Health Navigator at Te Mana Oranga Trust, you will play a pivotal role in improving health outcomes for the local community. Although this full-time position is based in Kaikohe, Northland, you will be responsible for supporting and guiding individuals and families to access the healthcare services they need. Much of this work could include picking people up from their homes in the Kaeo area and transporting them to Kaitaia, Kerikeri, Kawakawa or Whangarei for appointments.What you'll be doingAssisting clients to navigate the healthcare system and access appropriate services, including general practice, medical specialists, community health providers and social servicesProviding one-on-one support and advocacy to help clients overcome barriers to accessing careConnecting clients with relevant resources and programsCollaborating with healthcare providers and community organisations to coordinate holistic careMaintaining detailed client records and preparing reportsParticipating in team meetings and professional development activitiesLots of driving, picking people up for appointments so you will be one who loves to chatWhat we're looking forLevel 4 Mental Health and Wellbeing Certificate is the minimum qualificationDemonstrated ability to build rapport and work effectively with people from diverse backgroundsStrong communication and interpersonal skills, with the capacity to liaise with healthcare professionalsExcellent organisational skills and attention to detailCommitment to improving health equity and advocating for vulnerable populationsCultural competence and understanding of local community needsExperience working with sufferers of mental health and addictionsNew Zealand residents only - we will not be sponsoring anyone from overseasThe successful applicant will be required to undergo a Police checkAbout usTe Mana Oranga Trust is a not-for-profit organisation committed to improving the health and wellbeing of our local community.
