Mo tenei turanga mahi| About the Role As a Care Coordinator- Child Health, you will play a crucial role supporting the achievement of national health goals and targets led by Te Whatu Ora.
These are aimed at improving the health of our Tamariki and population, including protection from Vaccine Preventable Diseases.
This role will be instrumental in developing and implementing systems and processes that facilitate early and timely newborn enrolment (NBE) and timely 6-week check/immunisation events in general practice, with a particular focus on our priority population.
These events are key contributory measures to the timeliness and completeness of the Tamariki Ora immunisation programme.
Your responsibilities will include: Collaborating with the wider child health provider community to establish and nurture robust connections.
Facilitating access to immunisation schedule events and ensuring the collection of high-quality data.
Role Modelling person & whanau centredness, embracing the whanau ora approach in all interactions.
Empowering people & whanau to become active partners in their own health journey.
Promoting and enhancing collaborative efforts across health and social services related to Child Health.
Utilizing a population-based approach and risk stratification to focus care on those with the greatest need, reducing health inequities.

Nga pukenga me nga wheako | Skills and Experience Candidates will need to have experience in Primary or Community healthcare sector and be passionate about child health and improving child health outcomes.
Ideally, they will also have: An annual practising certificate.
At least 2 years post-graduate experience as a health professional.
A comprehensive understanding of the inequities in access and health outcomes in Aotearoa New Zealand.
A collaborative approach to working and be known for having great relationships with stakeholders.
Knowledge of the Te Tiriti o Waitangi and be an advocate of our Ka Hikitia strategy.
