Te whiwhinga mahi: The opportunity We are looking for a Waiora Manaaki / Health Coach to join the Pinnacle team in Taupo.
The Health Coach role sits within the Taupo Extended Care team and works collaboratively with primary care and communities walking alongside whanau to identify their priorities to improve health and wellbeing.
In this role you will walk alongside whanau to help facilitate wellbeing, connection with the right services and agencies that meet patient and whanau needs, whilst also helping to identify and reduce any potential access barriers.
This is a fixed term role covering maternity leave working 20 hours (0.5 FTE) per week.
He korero mou: About you You will have a high level of cultural competency.You will have a relationship approach to your work. You will value the importance of manaakitanga, and a collaborative approach with whanau.Hold strong connection to Tuwharetoa matauranga and well-connected relationships in the community.Have the ability to grow the use of Maori models of health and support wananga both in general practice and community settings.Have a passion for continual improvement and ongoing learning in your practice.Mo to matou ropu: About the team Pinnacle has made an explicit commitment to Maori and our communities to improve equity of health and wellbeing, in accordance with Te Tiriti o Waitangi.
