You will work alongside the clinical team and with other health services. You will support the whaiora with their shopping, daily activities, GP appointments etc. A mental health background is preferable.This is a permanent, full-time role, you will be working 8am – 4:30pm, Monday – Friday. You will start and end your shift at Harakeke House in Manukau, Auckland.Responsibilities include:Advocate and support whaiora to be well and thrive.Connect them with resources that support them in living well and assist them in building resilience.Help connect with their communities, building the relationships that matter most.Foster positive change for a more positive life.Provide medication support for our whaiora.Support Whaiora with their appointments.Understanding and commitment to principles of recovery.Current full NZ driver's licence.Current first aid certificate.Applicants must hold the New Zealand Certificate in Health and Wellbeing (NZCHW) or a recognised equivalent.Support Worker experience or similar.Sound knowledge and understanding of the primary care sector.Knowledge of, and demonstrable commitment to, the principles of the Te Tiriti o Waitangi.Closing Date: Monday, 25th November 2024.Please note, this role may close earlier if a suitable candidate is found.Contact: Roganee Moodley, Team Coach on ****** for any questions.For further information, visit our website www.pathways.co.nzPathways is committed to healthy lifestyles and actively supports a smokefree environment.
