We visit people in their homes and work in an intentional, goal-orientated way so that people can move to thrive independently of us. You could think of yourself as a well-being coach and will be managing your own caseload with the support of a Team Coach.You will be:Goal setting and building confidenceSupporting people with budgeting and independent daily living skillsAttending appointments and advocating for peopleAssisting people to connect with their communitiesSupporting people to take medication independentlyThis is a permanent, full-time position.
