This organisation has been around since 2002 and works with more than 13,000 disabled people and their families across the wider Auckland region from Mercer to Wellsford.
Working in the Community Engagement Team as a Support Facilitator, you will be providing support, conducting needs assessments, arranging support packages and working collaboratively with clients & families to ensure they are receiving the right services and support in the right time frames.
Responsibilities include:
Conduct detailed needs assessments and identify clients support needsDetermine and coordinate tailored support packagesAuthorise and coordinate access to funded and non-funded supportEstablish and maintain quality relationships with clients/whanau, and work collaboratively with teams across the organisationEnsure accurate documentation and record keeping is in line with policy and processesTo be a success, you will ideally have:
Minimum 2 years work experience in health, allied health, education or disability sectors and/or disability related tertiary qualificationValid full or restricted driving license required.Strong communication skills, particularly able to relate to a range of different people and diverse culturesComputer savvy and quick to learn new systemsConfidence, maturity, emotional intelligence and an empathetic natureExperience providing support & services for people with a range of physical, intellectual and sensory needsThis is a great opportunity to be part of this remarkable organisation that aims to make a difference in the lives of people with disabilities!
