Are you a highly skilled Specialist Frailty Practitioner looking to work within a Frailty team to deliver care to the residents of Dorset?Joining a team working in partnership with the local Primary Care Network (PCN) delivering the Ageing Well Agenda. You will be working with the team in service delivery, prioritising urgent visits and proactive care, to achieve our overall objectives in the delivery of the Virtual Ward, UCR and anticipatory care.The Specialist Frailty Practitioner will be based in the East Dorset Frailty team at St Leonards Hospital in St Leonards, Ferndown. You will join an excellent team of Advanced Clinical Practitioners, who are supported by two GPs and a long-term condition HCA. You will also become an active member of the East Dorset Hub, which offers an integrated service approach to the needs of patients, supported by social services, health and social care coordinators, rehabilitation/therapy services, community nursing teams and older persons mental health practitioners.Providing specialist clinical advice and support ensuring the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical analysis, and advanced decision-making skills. Your role will be community based, covering the East Dorset and Poole North localities.To provide a high-quality person-centred approach to care delivery which always considers people's safety, privacy and dignity using specialist clinical knowledge that is evidence based.Working as an autonomous practitioner to proactively assess, plan, implement and evaluate individualised programmes of evidence-based care as part of the wider multi-disciplinary team.To undertake independent assessment and management of service users to incorporate pharmacological considerations, where appropriate making complex clinical decisions regarding service user management and clinical outcomes.To utilise highly skilled and effective communication to facilitate appropriate and comprehensive advanced care planning. To devise specialist programmes of care for other professionals to deliver and ensure they are delivered appropriately.To support care home staff and residents providing specialist advice to patients, carers/relatives and the multi-disciplinary team. To provide education across the PCN and with partnership organisations.Provide supervision, mentorship, training and clinical support to students and trained practitioners.Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.
