About the Role Please note: Full Registration with NZSWRB is essential for your application to be successful Our client is seeking a Health Improvement Practitioner to join their general practice team. You will deliver brief, consultation-based services focused on evidence-based behavioural interventions for both mental and physical health conditions. This role is central to supporting an integrated 'Model of Care,' promoting seamless collaboration between general practice, community NGOs, and specialist services. Your care, empathy, and culturally responsive approach will help guide patients toward holistic well-being.
