About the Role You will play a key role in an intensive, personalised, Maori led and clinically underpinned response to the health, cultural and social needs of approximately 70 tangata whaiora and their whanau in the Waikato region. This group of tangata whaiora who have been identified as having high needs, and who are not getting these needs met by the current service system are predominately Maori (about 70 per cent). 'High need' has been defined as a mixture of mental health and addiction problems, physical health conditions, and disability combined with significant impairment and disadvantage across a range of social, psychological, behavioural, occupational and interpersonal domains.
Key responsibilities of the role: Navigating complex biopsychosocial situations using an ecological systems approach to assist with identifying and addressing barriers to accessing health/mental health care and community support services.Assessment of support needs to determine eligibility for mental health residential recovery beds.Provide holistic and comprehensive assessment of current state and guide plan to address these issues. These include support needs assessments, social inclusion, risk assessment and safety planning in the context of family harm (elder abuse, child protection and the safeguarding of vulnerable adults). Social worker to work in partnership with Whanau Advisor about building connections.Leading practice with service users/tangata whaiora who have dual disabilities and require support from other funding provider such as ACC and DSL.Providing advice and support to families regarding EPOA and PPP&R Act 1988 and a willingness to make application to Family Court if there is no other option to facilitate access to residential services or ensure protection as needed.About the Team Oranga Rangatira is committed to providing a well-integrated and cohesive clinical and cultural model of care that will ensure services provided to tangata whaiora and their whanau enhance their Oranga, Mana and dignity.
Our success is driven by the ability and willingness to liaise and work alongside Kaimahi across the Waikato region, internally and externally, to provide a flexible community-based multi-disciplinary, intensive treatment and rehabilitation service for tangata whaiora and their whanau.
About you Competency with te ao Maori, tikanga, and te reo Maori or a commitment to starting your journey and taking ownership of your learning and growth.Experience in upholding Te Tiriti principals and frameworks, and the application of Matauranga Maori and Kaupapa Maori approaches, particularly as they apply in healthcare settings.Full registration with Social Workers Registration Board with a current APC.Minimum of 5 years work experience working with "high need individuals and or whanau" experiencing significant mental health and addiction problems.A strong desire to work from a Te Ao Maori perspective, acknowledging contributions from both cultural and clinical perspectives.Working for Health New Zealand Health New Zealand | Te Whatu Ora is 'the weaving of wellness'.
