Job description Service Coordinator
Hours: Permanent, full-time (40 hours per week)

This role offers you an exciting opportunity to use your skills within the Wellington community which contributes to improving the health and wellbeing of disabled people.
This role coordinates services provided by CCS Disability Action. You will work in partnership with individuals and their families to develop, deliver and/or coordinate a range of person-directed services that aim to empower and enable the individual to achieve their personal lifestyle goals.

About the role:
Develop and maintain community connections and networks; and promote the concept of inclusion and community development.
Coordinate high quality services that are creative, flexible, and responsive to the needs of disabled people and their families/whanau.
Coordinate the delivery of contracted services.

We would love to hear from you if you:
Hold a relevant Degree or Diploma in Social Services or acceptable equivalent qualification and/or work experience.
Have experience working in a social services agency, and in particular working with disabled children/youth/adults, their families/whanau, and significant others.
Can demonstrate a willingness to be creative and innovative in delivering services.
Networking skills and knowledge of government, non-government, and private sector services available within the community.
Understanding of person and family centred planning and community development concepts.
Is committed to providing quality customer services.
Strong time management and organisational skills.
Excellent written and verbal communication and reporting skills.
