The Opportunity: This position is responsible for coordinating the provision of patient and whaanau-centred safe and effective Needs Assessment and Service Coordination within an interdisciplinary team setting.
You will be providing specialist assessment and support to residents of aged-care facilities, in primary care, and in the community within Counties Manukau District.
This will include education to NASC and Aged-Residential Care (ARC) facility staff to build capability and to support decision making relating to admission and hospital presentation avoidance, and to assist with facilitating timely discharge from hospital.
You will be using the InterRAI assessments including the DIVERT outcome scores to identify and analyse patients at risk of hospital admission and ED presentation.
Working collaboratively with the interdisciplinary team, you will provide comprehensive care planning for complex patients at risk of hospital admission, discharge delay, or re-admission.
You will support allocation and care planning informed by specialist knowledge and include support for the patient and family/whaanau across the continuum of care with a strong quality focus.
Essential: NZ Registered Health Professional with a current Annual Practising Certificate and Professional body registration such as a Registered Nurse, Occupational Therapist, Social Worker, or Physiotherapist. Minimum 5 years postgraduate experience in a relevant setting, including 3-4 years experience in the area of speciality. InterRAI assessment trained. Experience in teaching, mentoring, and coaching. Experience in leading innovation and quality improvement. Excellent verbal and written communication and computer skills. Full, clean NZ driver's licence.
