Tasks involved: Client Coordination: Provide excellent customer service to both Life Plus clients and staff - developing positive relationships Together with the Registered Health Professional (RHP) Consultant, onboarding new clients Entering new client referrals into the Alayacare system Receiving and acknowledging client extensions of care Reviewing client documentation from partner organisations to ensure accuracy and appropriate for client support needs Liaising with Partner organisations, other funders, and healthcare professionals regarding client service delivery Arranging carer training including client-specific training with MDT Developing and setting up client rosters based on support needs assessment, funding approval, and client preferences Troubleshooting as required for clients and staff Administration / Database Management / Coordination: Develop an understanding of the Alayacare system (training given) Rostering of staff schedules Approve staff visits and travel to clients Ensure staff documentation is complete and inputted into the Alayacare system Ensure the Alayacare system is maintained and up-to-date Ensure staff records are updated and maintained for currency of information Resourcing / Recruitment: Developing and posting adverts Screening candidates Booking and conducting interviews Completing reference and employment checks Booking orientations and training Organising competency assessments and appraisals for staff About you: Have healthcare Coordination experience or Coordination experience in another sector You will be self-motivated, reliable, flexible, and punctual Good verbal and written communication skills with the ability to foster relationships Good level of computer literacy Caring and sensitive to the needs of others Culturally aware An active team player but also able to work on their own initiative Have excellent computer skills and excellent attention to detail A complete multi-tasker Take your career to the next level and join a dynamic team that's growing by the second!
