About the role | He korero whakamarama mo tenei turanga
You will be working as part of our CREST Nursing service as a Case Manager to provide personalised community-based assessments to newly discharged clients with a restorative approach. This involves identifying and resolving issues as and when they arise to maximise rehabilitation potential. This short-term service aims to return clients back to their baseline level of function and if that is not possible, to plan for long-term support.
This role involves working with our established team of Nurses, Coordinators, Support Workers, and other Allied Health professionals within the community setting. As a Registered Nurse, you will provide a broad range of care to clients in their own home such as:
Passion to deliver exceptional services to people with health or injury-related needs in their own homes.
Demonstrated experience completing assessments
Ability to develop and review goal-focused service plans based on individual client needs
Excellent customer service
Ability to work autonomously and as part of a team
This is a full-time role working:
Monday to Friday 8am to 4:30pm
On-call cover once every 5 weeks

Strong communication and organisational skills
At least 1 year of experience as a Registered Nurse and have a current registration
Restricted or Full Driver's License - must be able to drive independently!
