Full-time role, Monday to Friday, 8:00 am – 5:00 pm

Responsibilities Semi-mobile position requiring travel between clinics, client residences, community hubs, and marae
Manage a spectrum of chronic illnesses with varying degrees of severity
Educate clients' families and support networks on managing chronic conditions and goal setting for improved well-being
Actively promote organizational services through referrals, social media, and other appropriate channels
Implement a tailored Whanau Ora support program, beginning from illness onset through to end-of-life care, as needed
Requirements Current APC and registration
Prior experience in chronic disease management or relevant postgraduate qualifications is essential
Possession of a full, clean driver's license
Strong communication and educational skills
While Te Reo proficiency is not mandatory, it is advantageous
Familiarity with medical operating systems like Medtech is beneficial, with training provided for new team members
