This is a regional innovative supportive service working with tangata whaiora in supporting their journey with a multi-disciplinary team approach.We advocate for best practice, sharing of ideas, and the continuum of change management processes to align with our vision.You will need to work collaboratively within a team; support Mental Health Support Workers (MHSW) under your direction; and assist MHSW's in undertaking care activities.You will bring your active listening skills and solution-focused approach to foster tangata whaiora in achieving their identified goals within their pathway journey. You will have the ability to learn new skills as required to support your role within the team, as well as good communication skills with your team members, tangata whaiora, and whanau. You will be proactive in ensuring all nursing-led documentation reflects a person-centred approach and promotes independence.
