Salary: As per NZNO MECA - Step 3– Step 8 ($76,554 - $95,340) depending on experience
Korero mo te turanga – About the role Older Persons Mental Health is predominantly a community based service. This position is for a Registered Nurse to join our community multidisciplinary team.
Mo to matou ropu – About our team We provide assessment, treatment and rehabilitation interventions for older adults experiencing recent onset functional mental ill health and/or behavioural and psychological symptoms secondary to degenerative neurocognitive conditions, primarily dementia. We also see older adults with complex clinical presentations where specialist assessment is indicated within our scope. We work closely with geriatric medicine and other mental health services across the district. You will be joining an experienced specialist team who are passionate about improving health outcomes and promoting the quality of life of clients referred to our service. You will be a team player and a valued member of the team.
Mou – About you You will bring your experience as Registered Nurse working in mental health to this position. Experience in this area of speciality would be an advantage. You will be expected to undertake specialist assessment and formulation, develop comprehensive treatment and intervention plans in collaboration with the client, their whanau and other key stakeholders, promote a range of psychosocial interventions, provide case management and coordination of care.
Your duties will include:
Provision of high quality nursing and psycho-social interventions through specialist assessment, treatment planning and review with a rehabilitative recovery focus for the client and whanau Coordination of intensive intervention and case management for clients in crisis when appropriate To undertake comprehensive specialist mental health assessments To provide case management and coordination of care for clients and their whanau Liaison with other health providers, mental health teams, geriatric medicine, NGO's, PHO's, social and government agencies to facilitate care and treatments for clients Participation and contribution to service planning and quality projects Assisting the team as needed included with triage processes as required E pa ana ki a tatou – About us Here at Capital, Coast & Hutt Valley District we embrace diversity and welcome applications from all people, including all gender identities, ages, ethnicity, sexual orientations, disability and religions.
