In this role, you will be facilitating the management of care by ensuring timely responses and a smooth transition for our clients along the treatment pathway. You will conduct risk assessments, facilitate treatment care planning, provide appropriate health education, and evaluate clients' progress and outcomes against treatment goals. This is a dynamic and supportive service evolving to meet community needs and supports team members to reach their full potential.
