We aim to support consumers that requireextended assessment or treatment following an admission to the acute inpatientservice prior to discharge. This may include: * tangata whaiorawho are presenting to services for the first time that may require extendedassessment and support around management of their symptoms. * tangata whaiorathat have an identified discharge pathway on admission - i.e. home or supportedaccommodation - but may require some additional assessment and upskilling tosafely and successfully transition into the community. * tangata whaiorawho may have limited social supports and require support to increase theirengagement in their local communities.
