Primary Mental Health Clinical Co-ordinator (0.6 FTE)Waitaha Primary Health (WPH) has responsibility for the provision of primary health services to approximately 50,000 enrolled patients in Canterbury and contracts with Te Whatu Ora and several other health funders.WPH is committed to reducing health inequalities for its enrolled population and works with health care clinicians, health care providers, Iwi and local communities to improve health outcomes.Christchurch PHO (CPHO) has responsibility for the provision of primary health services to approximately 60,000 enrolled patients in urban Christchurch.This person will be employed by WPH to deliver a clinical co-ordination service to CPHO. The purpose of this position is to co-ordinate and lead a team of dedicated mental health practitioners that are contracted directly by Christchurch PHO. This role will require hands-on involvement in triage, referral allocation and delivery of services, clinical leadership, and providing peer support to clinicians within the team.The Clinical Co-ordinator will develop a close working relationship with GP referrers, community and NGO providers and take responsibility for the clinical schedule of the CPHO clinicians. This is also a great opportunity to move into a team leadership/co-ordination role.
