Enhancing patient health outcomes by optimising medication use through a comprehensive approach to medicines management, clinical evaluation, and reconciliation. This includes reducing medication-related harms, ensuring continued appropriateness of therapy, and supporting patient and whanau understanding of treatment. Additionally, the opportunity exists to improve care for high-risk medications and at-risk populations, while also driving quality improvement through system-directed activities such as pharmacotherapy advice, medication audits, and policy development for safer and more effective medication management across healthcare settings.
Duties will include: Medicines management and optimisation to improve medicines-related health outcomes through optimising medicines use (medicines optimisation/management review) Clinical evaluation and management of medication use Medication reconciliation Advise and support whanau understanding and experience of medication treatment, how it can affect their health and wellbeing – including lifestyle, impacts on adherence, and issues that may occur with administering medications, including problematic polypharmacy Supporting management of long-term conditions through initiation and continuation of prescribed treatments within area of practice (Prescriber scope only). Improving management of high-risk medicines e.g., lithium, warfarin and in at-risk populations e.g., diabetes, dementia, and during transitions of care Kowai koe?| About you You'll bring to the role: Bachelor of Pharmacy (BPharm) or equivalent overseas qualification Qualified pharmacist with current APC without restrictions Pharmacists undertaking prescribing activities must be registered and have a current APC (without restrictions) in the Pharmacist Prescriber Scope of Practice. Holds, or is working towards, relevant postgraduate clinical pharmacy qualification (preference to Postgraduate Diploma level or equivalent) Experience in implementing Te Tiriti o Waitangi in action Primary or community healthcare experience Be careful - Don't provide your bank or credit card details when applying for jobs.
