Job summary The post holder isresponsible for the delivery of basic practice nursing services care to thepractice population.
Supported by the GP Partners and nursing team within thepractice, they will deliver care within the boundaries of their role, focusingupon supporting patients to be healthy, monitoring of long-term conditions,health prevention and screening activities.
They will work collaboratively withthe general practice team to meet the needs of patients, supporting thedelivery of policy and procedures, and providing nurse leadership as required.
Main duties of the job To practise a range of clinical skills gained through previous experience, undertaking the delivery of nursing care as a self-directing practitioner, working with the GPs to meet the clinical needs of our patients as appropriate.To support structures already in place for the smooth running of the practice.To provide effective leadership and management skills.To ensure own actions are consistent with clinical governance systems.To conduct audit and patient screening in pursuance of the objectives of the Quality & Outcomes Framework (QOF) of the PMS contract.
