Salary band: $77,087 - $107,600 + Kiwisaver + on-site employee gymMo te turanga | About the roleThe Home Team is an interprofessional team designed to support discharge from hospital or prevent unnecessary hospital admission through rapid allied health and nursing assessment and support at home.This service works in partnership with clients and key stakeholders, using frailty as an indicator.
The Home Team plays a critical role in supporting the Medical Assessment unit.
This is a fast-paced work environment where patients, admitted from ED or directly from the community, are comprehensively assessed to determine if ongoing inpatient care is required or if the patient can be discharged home.Key Responsibilities:Support the early assessment, intervention, and coordination of services to improve patient outcomes and patient flow within our hospital.Demonstrate advanced comprehensive clinical assessment and problem-solving skills.Direct and delegate to Rehabilitation Assistants.Communicate effectively with health professionals within inpatient services and the community setting.Work with older adults in acute, rehabilitation, or community settings.Minimum Requirements:Registered Health Professional with a current APC, ideally with 2 years post-registration experience in an appropriate setting.Full clean NZ Driver's License.Ability to adapt to change and be flexible within the working environment.Benefits:Beautiful coastlines and a family-friendly community.Continuous support for career development.Subsidised gym membership and discounted health insurance.Access to staff fitness facilities and banking offers.Access to a range of retail discounts and Employee Assistance Programme (EAP).Health New Zealand | Te Whatu Ora - Southern has the largest geographical coverage of all the New Zealand regions, providing services to 350,000 people in diverse communities.
