The Pathway Coordinator is responsible for oversight and coordination of the CDS client pathways. This includes clinical triage of referrals received and resource management and planning to ensure best results for clients and service. The coordinator provides support to the line manager by taking on delegated leadership and operational tasks for the team. This role may also be required to provide direct clinical care, as appropriate to the needs of the service area.
Child Development Service (CDS) is a multidisciplinary team, working together to provide specialist assessment, diagnosis, and intervention for pepi, tamariki, rangatahi with disabilities and/or developmental delays. The team includes Speech-language Therapists (SLTs), Visiting Neurodevelopmental Therapists, Occupational Therapists, Physiotherapists, Psychologists, Social Workers, Pathway Coordinators, Developmental Paediatricians and ASD coordination. These disciplines work closely to provide integrated care in a transdisciplinary way, using a holistic approach to enable children to reach their maximum potential within their whanau, hapu, and environments.
This role requires an Allied health professional with a commitment to supporting children and families to access services to support their needs and liaising with a multidisciplinary team to deliver a high quality equitable service across our district.
You will be motivated to provide leadership support across the team and take on delegated operational leadership tasks that support the team leader.
